NHS South West London have paid to use the Universal Care Plan (UCP) product. BetterCare are the system suppliers of UCP.
As of August 2023 there were 5000 active care plans for patients across 22 different practices within London.
The Universal Care Plan is an NHS service that enables every Londoner to have their care and support wishes digitally shared with healthcare professionals across the capital. A care plan is created following a conversation between a healthcare professional (such as a doctor or nurse) and the person in their care.
Previously, agreed care plans were only available to clinicians and healthcare practitioners. By integrating with the NHS App, Universal Care Plan will allow citizens to view their care plan (they will not be able to edit). The NHS App integration will be the only route for patients to view their Care Plan.
You can use UCP to:
- access care plans that include how you would like a professional to contact you, current needs, social situation, any lasting power of attorney documentation (ability to download this also), support network, advanced decision to refuse treatment, prognosis and preference of location of care, urgent care plan info, symptom management plans, and medicines and allergies.
- Please note, that the info stored in UCP comes from UCP and not from PDS. This means there may be some discrepancy of information between what is shown in areas of the UCP and elsewhere in the NHS App. This is because the data source is different.
How to access UCP:
Accessed via the "Your Health" section of the NHS App, then by selecting "Care plans" tile.
- If a user has a care plan with another provider (e.g. PKB), they will be shown all care plans that they can view. If they select UCP, they will then be taken to the interruption screen.
- If a user only has UCP, they will go straight to the interruption screen to take them directly to UCP.
- If a user's practice uses UCP but the user has no care plan themselves, they will be shown a screen that advises them they have "no care plan yet"
Once the user continues into UCP after the interruption screen, they will enter UCP's product. If they have a care plan they will see the screen with the details there. If they do not have a care plan, they will see a screen informing them of this.
