A personal health record stores information about your health and care online. It's managed by yourself, and health professionals can add information to it.
Your GP surgery or hospital contracts with one or more providers to deliver personal health record services. The providers are commercial organisations. They provide a service on behalf of your surgery or hospital.
We currently support personal health record services with Patients Know Best Ltd and Substrakt Health.
If your GP surgery or hospital offers personal health record services for the NHS account, you may see any of the following options:
- consultations, events and messages
- hospital and other appointments (view appointments)
- shared links
- care plans
- hospital and other medicines
- track your health
- record sharing
- update your personal details
- ask your GP surgery a question
- patient participation group
You can find more information in the NHS App personal health record services privacy policy.
